What is an Affidavit? It is simply a formal statement of fact and can be used for many different purposes. The notary public witnesses the signing of documents, authenticating the identity of the signer. Ours requires you to confirm the following.
1.) I am a U.S. citizen.
2.) I am at least 21 years of age.
3.) I do not have any history of Mental Health issues.
4.) I do not have any felony convictions, misdemeanor domestic violence convictions, under indictment or investigation.
5.) I am the End‐User of the Body Armor product(s) ordered through TYRtactical.com.
6.) I will not export any Body Armor product(s) ordered through TYRtactical.com.
7.) I am familiar with my current local and state laws pertaining to Body Armor purchase and ownership, and there is no legal restriction against my purchasing or owning Body Armor products.
8.) I do not intend to commit a crime using Body Armor product(s) ordered through TYRtactical.com.
We do not perform Background Checks and the affidavit is not kept in any public record. It is safely stored at our flagship location in Peoria, Arizona.
Where do I get something notarized? Your bank or credit union typically has a notary on staff. Many banks, such as Bank of America, offer the service for free to their customers.
How do I submit my Affidavit?
